Checks are made payable to the "Jackson Soccer Club" unless otherwise noted.
Jackson Soccer Club (JSC) Travel Soccer Registration requires a combination of forms from the JSC and New Jersey Youth Soccer (NJYS). The combined forms are listed below with associated instructions.
All Players and Parents complete these forms and submit them to their Coach:
Players, Open and Read the Player Code of Conduct, then Sign the JSC Travel Registration Form.
Parents, Open and Read the Parent Code of Conduct, then Sign the JSC Travel Registration Form.
Parents, Open and Read the JSC Select Travel Policies, then Sign the JSC Travel Registration Form.
Parents, Provide a Copy of Player’s Birth certificate
Parents, Complete and submit the Jackson Soccer Club Travel Registration Form
Fees: Players playing in Ocean County Rec require a $75 check for the registration fee. All other travel team players require a $100 check for payment of Fall 2010 Season. There is a $260 Family Cap with reference to travel only, recreation fees do not count toward the travel Cap limit.
Registration payment is required for all coaches who have a player on the team.
Parents, Complete and submit the Volunteer Work Form (requires an un-dated $100 check. Be careful to sign the Donation section ONLY if you plan to donate the fee to the soccer club instead of a volunteer work assignment)
When Coaches Complete the Volunteer Work Form for your child, you need to note yourself as a coach, but you do not need to provide a $100 check. Only coaches who are officially rostered and carded with the team for NJYS are exempt from additional Volunteer Work.

Coaches complete the following forms and submit with the team package:
Submit the Applications to Travel forms from tryouts (if not already provided).

Required Information for NJYS Cards (Not for U7 Ocean County Rec teams):
Coach or JSC Commissioner will Complete the Official State Roster on the new NJYS online registration system.
Player passes will be distributed in the pass packet for printing. Coaches will: Have the passes signed by players, add the player pictures, and submit to the Overall Commissioner for approval by the NJYS DC.

Coaches who coach more than 1 team must be added to the roster for each team that they coach, using the player pass number from their primary team pass. NJYS Add Forms are no longer required.
After all of the players/coaches passes are printed, signed, and photos attached, they will be submitted to the Overall Commissioner with the NJYS forms for processing by the DC. ALL FORMS WILL BE IN ALPHABETICAL ORDER by Form Type (i.e. All Birth certificates n order, all player member forms in order, etc. etc.).

NOTE: Teams Playing only MAPS or JAGS will submit their player passes to the DC for those Leagues. A copy of the final Official Roster must be submitted to the Overall Commissioner for our Records.
US Club Cards
Contact the Overall Commissioner ahead of time if you plan to apply for US Club Cards.

To apply for cards, you will need a complete list of players to be rostered which contains: Name, Address, Phone, DOB, email, Jersey #, Electronic copy of Birth Certificate, Player Registration - Medical Authorization Form.
Coaches Passes are good for a period of 2 years. If you require a new pass or are doing this for the 1st time, All US Club Coaches must complete the Online Staff Background Check Application before they will be carded.
