Jackson Soccer Club Travel Registration Form
Teams playing in Ocean County Rec require a $65 check for the registration fee. All other travel teams require a $90 check for payment of Fall 2008 Season. There is a $240 Family Cap with reference to travel only, recreation fees do not count toward the travel Cap limit.
Registration payment is required for all coaches who have a player on the team. Complete the Volunteer Work Form and note yourself as a coach, but you do not need to provide a $100 check. Only coaches who are officially rostered and carded with the team for NJYS are exempt from additional Volunteer Work.
Volunteer Work Form (requires an un-dated $100 check. Be careful to sign the Donation section ONLY if you plan to donate the fee to the soccer club instead of a volunteer work assignment)

To apply for cards, you will need a complete list of players to be rostered which contains: Name, Address, Phone, DOB, email, Jersey #, Electronic copy of Birth Certificate, Player Registration - Medical Authorization Form.
All US Club Coaches must complete the Online Staff Background Check Application before they will be carded.
