When submitting the paperwork to ADD a Coach, you need to provide:

The NJYS Volunteer Coach Membership Form for the coach, signed by the coach

Coach’s Pass with picture attached and signed by the coach

Copy of NJYS coaching license

Kids Safe Form

3 copies of the updated NJYS Team Roster (showing the Added coach)

The old Offical NJYS Team roster, stamped by the DC

All JSC paper forms for coaches

When submitting the paperwork to RELEASE a player or coach, you need to provide:

Completed and Signed NJYS Release Form for player or coach, signed by the player/coach (3 copies)

The Player’s Pass

3 copies of the updated NJYS Team Roster (showing the Released player)

The old Official NJYS Team roster, stamped by the DC

There are no JSC Refunds when a player is released from a Team after the 1st game has been played

When submitting the paperwork to TRANSFER a player to your team, you need to provide:

There is no longer a Transfer Form for NJYS

The paperwork to ADD the player to your team (see above)

The final stamped/approved copy of the player’s RELEASE Form from the other team, if they were rostered to another team during this soccer year (August 1 - July 31)

3 copies of the updated Offical NJYS Team Roster (the new form will show the player Added)

The old Official NJYS Team Roster, stamped by the DC
